This goes straight to the person who maintains the tools. If you have spotted something clinically wrong, a dose, a threshold or a step that does not match current guidance, that is the most useful message you can send, and it is read first.
Please do not include any patient identifiers. Describe the problem, not the patient. This form is about the tools themselves, and not a route to advice on a patient you are treating.
Only if you would like a reply. Leave both blank and the message is still read, but there is no way to answer it. An address is used for nothing else and is never shown to anybody else.
