This is a published Indian score. It asks four things: your age, your waist, how much your body works in a day, and whether your parents have diabetes. It does not test your blood, and it cannot tell you whether you have diabetes. What it tells you is whether a blood sugar test is worth having now.

The score asks for the waist rather than the weight. Fat carried around the middle is the kind that affects blood sugar. The button turns on once your age, sex, waist and all three questions have been answered. The Indian Diabetes Risk Score was built in Chennai. It was published in 2005 by V Mohan and colleagues at the Madras Diabetes Research Foundation. The paper appeared in the Journal of the Association of Physicians of India. It came out of the Chennai Urban Rural Epidemiology Study. In that study 2,350 people had a glucose test. They were also asked about their age, their waist, their daily activity and their parents. The four things that predicted diabetes best were kept. The weights from that analysis were scaled so that the highest possible total is 100.
It was made for a particular job. That job is finding people who already have diabetes and do not know it, without testing everybody's blood. That is why it is short. That is why it needs no laboratory. And it is why the only thing it can honestly tell you is whether to go and be tested.

Under 35 adds 0. From 35 to 49 adds 20. Fifty or above adds 30.

Regular exercise and hard physical work together add 0. One of the two adds 20. Neither, with a sitting job, adds 30. The original table has these three lines and no fourth. A four-line version with a middle step worth 10 points is widely reproduced elsewhere and is not this score.

Neither adds 0. One parent adds 10. Both add 20. Nobody else in the family counts towards the score.

Every point in the score is a multiple of ten, so every total is a multiple of ten. There is no way to score 47 or 63.

The paper gives one cut-off and one only: a score of 60 or more. At that line the score found 72 or 73 of every 100 people in the study who had diabetes without knowing it. It also sent a lot of people for testing: 43 of every 100 scored 60 or more. Of those sent, about 17 in 100 turned out to have diabetes.
The three bands this page prints are under 30, 30 to 50, and 60 or more. That is the way the score is usually grouped in Indian screening work. They are not in the original paper, and we have not found the source that first set them. They are used here because a graded answer is more useful than a yes or no. They are named as a convention rather than as part of the instrument.

This is the most important line on the page. In the study itself, roughly one in four of the people who had undiagnosed diabetes scored below 60. Age carries the most points, so a person under 35 can do everything wrong and still score low. If you have symptoms, or your doctor wants a test, the score does not overrule either of them.
A high score is not a diagnosis either. Most people who score 60 or more do not have diabetes. It means a blood test is worth doing, nothing more.

All of the figures above were measured in one city, Chennai, in the early 2000s. Two of them depend on how common diabetes is in the group being screened: the 17 in 100, and the odds that a low score is right. Those will read differently in a village, in a young workplace, or twenty years later. The score has since been used in many parts of India, including Karnataka, and it holds up as a screening tool. It has not been rebuilt for today.

The arithmetic is the published one, unchanged. We have not added a factor, moved a cut-off or reweighted anything. If we had, the score would no longer be the Indian Diabetes Risk Score and it would be wrong to print that name above it.
Two things on this page are ours and are marked as ours. The question about symptoms adds no points and changes only the advice. The suggestions about your waist and your daily activity are ours as well. They follow WHO activity targets and the Indian waist cut-offs rather than anything in the 2005 paper.
